Urology screening

Mac with Cipro

Hardy's MacConkey with Cipro was designed by Dr. Liss, a urologist who saw the need for a way to screen his prostate biopsy patients for resistant bacteria before the procedure. By detecting these resistant gram negative bacteria, appropriate prophylactic therapy could be initiated prior to the biopsy in order to prevent septicemia.

HardyCHROM
Vibrio

At last, there is a selective media for Vibrio that will distinguish between all three of the clinically significant pathogens:
  • V. cholerae
  • V. vulnificus
  • V. parahaemolyticus

These three species will react differently to the chromogens in the media and to UV light shined on the plate.

Phraseology

"Dead as a Doornail"

This one goes way back...in fact the phrase first appeared in literature in 1350. By the time Shakespeare used it in King Henry VI in 1592, it was widely used and understood.

In medieval times doors were very heavily constructed of horizontal and vertical planks.

To secure the door's planks, doornails were pounded through both the horizontal and vertical boards, and to provide added strength and to make sure the nails would not fall out over time, the exposed shafts of the nails would be bent back and pounded into the crosspieces.

We must realize that in those days all metal objects, including doornails, were handmade and relatively expensive, and every attempt was made to constantly recycle and reuse these objects. But because bent doornails were difficult to salvage and unbend, they had no afterlife. They were considered "dead." Dead as a doornail...and hence the expression that is still in use today.

Can you do a Gram Stain without a microscope?

Lanagram

Yes there is a way to determine the Gram reaction biochemically without having to perform the stain. Hardy's Lanagram tests for the Gram reaction by using aminopeptidase. Lanagram is especially useful for "hard to determine" bacteria and can be finished in as little as five minutes.

The tube on the left shows a Gram negative reaction. The one of the right shows a Gram positive reaction.

Listeria Detected in Ice Cream

According to the CDC, an estimated 1,600 illnesses and 260 deaths occur every year in the United States due to listeriosis. In March of this year, the CDC reported an ongoing outbreak of Listeria monocytogenes infections that started in January 2014.

The outbreak consists of two clusters of illnesses that are linked to Blue Bell brand ice cream products. There have been a total of eight cases involving hospitalizations and three deaths in two states.

The first cluster of illnesses was reported from January 2014 through January 2015. All patients were from Kansas and all were hospitalized at a single hospital for unrelated problems before onset of listeriosis. Four of the five patients had consumed milkshakes made at the hospital with a Blue Bell brand ice cream product called "Scoops." Listeria monocytogenes sequences isolated from these patients were highly related to sequences of Listeria strains isolated from "Scoops" and two other products made on the same line at Blue Bell Creameries' Texas facility.

All products involved were recalled by Blue Bell Creameries on March 13, 2015. A list of all recalled Blue Bell Creameries products can be found on the CDC's website .

The three remaining patients were reported from Texas during 2011 through 2014. Like the patients from Kansas, they were all hospitalized before becoming ill. The sequences from the Listeria monocytogenes strains isolated from these patients closely matched strains isolated from ice cream manufactured at Blue Bell Creameries' Oklahoma facility.

Listeria monocytogenes is a ubiquitous organism and an intracellular pathogen. Notable virulence characteristics include its ability to replicate within host phagocytic cells and to spread from cell to cell by utilizing host actin. Consuming food contaminated with the bacterium is the most common way to acquire listeriosis. At least 90% of listeriosis patients belong to a higher risk group that includes pregnant women, adults 65 and older, and people with weakened immune systems. Listeriosis can have especially devastating consequences in pregnant women and can lead to miscarriage, stillbirth, premature delivery, or life-threatening infection of the newborn.

L. monocytogenes , unlike most other foodborne pathogens, can grow at refrigeration temperatures, making it a particularly important pathogen for manufacturers of ready-to-eat type foods such as deli meats and hot dogs where post-process contamination can be an issue.

Listeria has also been found in uncooked vegetables, unpasteurized milk and dairy products made from unpasteurized milk, certain soft cheeses, and smoked seafood. Foods linked to outbreaks of Listeria in the past have included cantaloupes, ricotta and other soft type cheeses, mung bean sprouts, and apples intended to be used for a prepackaged caramel apple product.

L. monocytogenes colonies turn a turquoise color
on the selective HardyCHROM Listeria plates.

L. monocytogenes is a significant public health concern and should be detected routinely where contamination can lead to risk of spreading the bacterium and causing infection. Isolation and detection of Listeria in food is recommended to be done with products such as Buffered Listeria Enrichment Broth (BLEB), LPM agar (Lithium Chloride Phenylethanol Moxalactam), Modified Oxford Medium, or PALCAM agar. In addition to these widely used products, Hardy Diagnostics also offers HardyCHROM Listeria (Cat No. G317), a chromogenic and selective medium for isolation of Listeria spp .from food and environmental samples, and Compact Dry LS (Cat No. LS100), a shelf stable, sample-ready medium, for the isolation, presumptive identification, and enumeration of Listeria spp. from food and environmental samples.

New Reference Guide for

Clinical Microbiologists!

Two new reference guides released!

Gideon Guide to
Antimicrobial Agents, 2015

Useful and practical  information on 303 anti-infective drugs, at your fingertips!

As of 2015, the fields of Clinical Microbiology and Infectious Disease are challenged by over 3,000 species of bacteria, viruses, fungi and parasites. 303 drugs are currently available to battle these pathogens, each with a unique pharmacology, spectrum of activity and testing standards and range of toxicities.

Gideon Guide to Antimicrobial Agents, 2015 contains 875 pages as a 10.95 MB PDF file.  Since this is an e-book, users can rapidly access a specific drug by name or trade name (currently 25,000  listed) using standard search tools.  The text includes drugs which are available in only one or two countries, and rare historical content on agents which have not been used since the 1950's.

The first of three sections in the book lists drugs by class and indication; the second is an A-to-Z guide by Disease name; and the third an exhaustive description of each individual agent: Mechanism of action, Typical dosage for adult or child, Details of metabolism, elimination and penetration into various body fluids, Toxicity (185 forms of toxicity), Drug-Drug interactions (558 interactive drugs), Spectrum (including Disc zone diameter and breakpoint standards) and Trade names.

Gideon Guide to
Medically-Important Yeasts, 2015

The clinical and laboratory characteristics of 119 yeast taxa associated with human specimens.

The book is an A-to-Z reference that summarizes the practical features of all yeasts associated with human medicine.  This exhaustive text includes distinctive laboratory characteristics, ecology and clinical significance, previous taxonomic designations ("synonyms") and full phenotype (biochemical reactions).

As of 2015, 119 yeast taxa have been associated with human disease.  The advent of Intensive Care and expanding range of immune suppression (AIDS, transplantation, cancer chemotherapy) has contributed to an expanding range of yeast pathogens and disease syndromes.

Gideon Guide to Medically-Important Yeasts, 2015 contains 131 pages as a 1.08 MB PDF.  The text includes taxa which have been described in only a single country, or even a single patient.  For each taxon, the ecology and disease association are described, followed by typical phenotype and drug susceptibility pattern, and a listing of older taxonomic designations (921 names as of 2015).
