Adaptations increase pathogenicity...

Bartonella
 
Bartonella is a bacterial genus capable of causing several human diseases. The three most common are cat scratch disease, caused by B. henselae; trench fever, caused by B. quintana; and Carrión's disease, caused by B. bacilliformis



Cat scratch disease can be transmitted from the scratches of cats, particularly kittens, due to cats harboring infected fleas that carry Bartonella. Most cats carry the bacteria in their blood without getting sick and some studies have found that 1 out of 3 healthy cats carry Bartonella in their blood. 

Trench fever is transmitted by the human body louse and is most commonly associated with homeless populations or areas of high population density and poor sanitation. In a recent analysis, it was found that about 33% of body lice recovered from infested homeless people were PCR positive for B. quintana

Carrión's disease is transmitted by bites from sand flies that carry the organism. These infections are endemic in the Andes Mountains in western South America, including Peru, Colombia, and Ecuador. 

Although different Bartonella species have different vectors of transmission and cause different types of infections, what they all have in common is the ability of the Bartonella bacteria to adapt to vastly different host environments. How are these bacteria overcoming stressful changes in their environment, and can the answers to this question lead to development of specific treatment for Bartonella infections?

Dr. Jane Koehler at UC San Francisco has studied the transmission of Bartonella quintana and has found temperature-specific regulation of genes that may confer survival of the organism in its two different hosts, humans and body lice. B. quintana must undergo change in temperature between hosts and a drastic change in hemin concentration between hosts. The louse vector has an internal body temperature of 28°C and very high hemin concentrations in the gut. In contrast, human host temperature is 37°C and the human bloodstream is hemin-restricted. Although hemin is necessary for Bartonella to obtain an iron source, the levels of hemin contained in the louse gut are at toxic levels, producing reactive oxygen molecules that can be lethal to bacteria. B. quintana must contain mechanisms that allow it to survive when exposed to decreased temperatures and increased hemin concentrations in the body louse gut.

Bartonella produces a family of hemin binding proteins (Hbp) that are responsive to temperature, hemin concentration, and oxidative stress. They also produce hemin utilization (Hut) proteins that are involved in hemin sensing, degradation, and storage. Both Hbp and Hut proteins are most likely important players in Bartonella's ability to survive in a wide range of hemin concentrations.

Wild type B. quintana has greater tolerance of high levels of hemin than what was observed previously for B. henselae. The difference in hemin tolerance displays how evolutionary adaptation allows each Bartonella species to adapt to its specific arthropod vector. Understanding what is essential for Bartonella survival and targeting the vector stage is important for interruption of the body louse to human transmission cycle.
